USPS Priority Mail with Signature Confirmation sent to a PO Box requires the recipient to sign for the package at the post office counter. A notice (PS Form 3849) is placed in the PO Box, instructing the recipient to pick up the item within 15 days by showing ID.

- Process: Instead of direct delivery, a notification slip is left in the box.
- Pickup: The recipient must visit the retail counter during business hours, present ID, and sign for the package.
- Storage: Items are generally held for 15 days.
- Electronic Option: Customers can use USPS Electronic Signature Online (eSOL) via Informed Delivery to sign for eligible packages digitally.
- Carrier Actions: If the box is part of a caller service, the agent may sign for it upon delivery, otherwise, it follows the standard slip-and-pickup process.
It is important for the recipient to bring the notification card and a valid photo ID to the post office.
